Prescription Savings Benefit

Members can either contact Catholic Knights Member Services department to request a card, or they
can go to www.scriptsave.com to enroll and print out a card. They will log-in using Group #823. They
can begin using the card immediately for savings on prescription drugs for all members of their
household.
 

The ScriptSave® card provides them with:

• Savings on brand name and generic prescriptions. Their ScriptSave® card can be used by all members of their household to fill prescriptions with no limit to the amount of use.
• A network of over 53,000 participating pharmacies nationwide for unlimited access, including chain,independent and retail pharmacies.
• Friendly and knowledgeable Customer Care Representatives accessible through an 1-800 number, to answer their questions about their prescriptions, specific pharmacy locations, and other healthsavings programs.

Savings will vary depending on the prescription and the location of the pharmacy; however the average
savings per prescription is 22%, with potential savings up to 50% (based on national program savings
data).
 

More than likely, members can continue to fill and refill their prescriptions at the pharmacy they currently use. ScriptSave® is honored at a nationwide network of over 53,000 pharmacies, including local independent pharmacies, major chain, supermarket and retail pharmacies across the United States. So even when they travel, the ScriptSave® card can be used to receive savings.
 

In addition to savings on prescriptions, members can use their card for savings on:

Vision care

• 28% savings on prescription lenses and frames (complete pair)
• 15% savings on conventional (non-disposable) contact lenses,
• 20% savings on non-prescription lenses and frames (complete set)
• $55 eye exam with dilation.

There are no limits on usage and members can save at over 13,000 locations including both private practice providers and optical retailers such as LensCrafters, Target Optical, Sears Optical and most Pearle Vision locations. Vision Care is provided thru EyeMed (contact ScriptSave® at 1-800-700-3957).

Hearing care

• 15% savings on over 70 hearing aid models at 1,350 preferred providers
• Free, 10-step hearing evaluation to diagnose the extent of hearing loss
• Lifetime cleanings, inspections, adjustments and service for any Beltone hearing instruments purchased

Diabetes care and supplies

• 5% savings on Envoy Health’s full line of name brand and generic diabetes testing supplies delivered directly to their home

If members do not have health insurance, they can use their ScriptSave® card for any brand name or generic prescription they or a family member pays for out-of-pocket.

If members have healthinsurance coverage, they can use their ScriptSave® card for any prescriptions that are not covered by their insurance or for savings for their family members not covered by insurance.
 

Members enrolled in Medicare Part D can use their ScriptSave® card for prescriptions that are excluded under Medicare Part D law.
