To change your address in regard to your Catholic Knights policy, please contact us with your member or policyholder's name and member number, if available.
To change your beneficiary, your name or to transfer ownership of your policy, complete a Change of Name, Beneficiary or Transfer of Ownership form.
With any requested-transaction form being submitted by a Guardian, a Guardian Certification form is required.
With every requested-transaction form being submitted by a Power of Attorney agent, a Catholic Knights' Power of Attorney Certification form is required.
To report the death of a loved one, please provide the insured's name, date of birth, date of death, policy numbers and the contact information.
To receive a duplicate of your policy, complete and mail a Duplicate Policy Request Form to Catholic Knights.
To request a policy withdrawal (loans, dividends, premium deposit fund, participation credit or annuity), please mail in completed Policyholder Service Request form.
